Intake Form & Widget
How to embed the callback form on your website and what homeowners see.
The intake form is how homeowners submit complaints about their inspection. It lives at a unique URL for your company and can be embedded directly on your website.
Installing the form
Your form is hosted at /widget/[your-slug]. There are two ways to use it:
- Iframe embed — Copy the embed snippet from Settings > Intake Form (or from onboarding step 6) and paste it into your website’s HTML. The iframe auto-resizes to fit its container, so it works in sidebars, full-width sections, and modal dialogs.
- Direct link — Link homeowners straight to the hosted URL. Useful for email signatures, printed materials, or anywhere you can’t embed an iframe.
What the homeowner fills out
The form collects everything Callback HQ needs to analyze a claim. Fields appear in this order:
- Name (required)
- Phone (optional)
- Email (required)
- Inspection address (required)
- Property ownership — The homeowner selects whether they are the sole owner or if there are co-owners. If co-owners exist, the form lets them add each co-owner’s name and email.
- Three screening questions (yes/no, all required):
- Is the claimant a minor?
- Is the claimant represented by an attorney?
- Has a lawsuit been filed?
A “yes” answer to any of these triggers a hard blockon the claim — the workflow halts entirely. See Reviewing a Claim for details on hard blocks.
- Inspection date (required)
- Inspector (optional) — If you have active inspectors on your roster, they appear in a dropdown. Inactive inspectors are shown greyed out and labeled “former.” If no inspectors are configured, this field becomes a free-text input.
- Issue description (required, max 3,000 characters) — The homeowner describes the problem in their own words.
- Which inspection is this about? (optional) — One pill per service you have switched on under Settings > Services. As the homeowner types their description, the likely service is pre-selected; they can pick several or leave it blank. This field only appears when you offer more than one service. When answered, the matching is deterministic: the named service’s report becomes the primary report for the claim.
- Work status (required) — One of three options:
- Not yet — no repairs started
- Got a quote — the homeowner has a repair estimate
- Work completed — repairs are already done
- File uploads — Up to 10 files, 25 MB each. Accepted formats: PDF, JPEG, PNG, HEIC, HEIF, WebP, GIF, MP4, QuickTime, and WebM.
After submission
The homeowner sees a confirmation screen with a reference number (the claim’s public ID). They can use this number if they need to follow up.
The claim immediately appears in your Inbox, where the AI begins analysis.
